- Fire Service Casualty – ‘Provides details of a fire service casualty’- Mandatory, minimum 1 to 100000
- Fire Service Casualty – Equipment Failure – ‘Provides details of the equipment that led to the a fire service casualty’- Mandatory, minimum 1 to 200
